Authorization and Acknowledgment: By completing this registration, I, the legal parent/guardian, grant permission for me and/or my child(ren) to fully participate in any Eastridge Church event and all activities congruent with their physical abilities. I recognize and acknowledge the inherent risks that these activities may present for me and/or my child(ren). By submitting registration, I DO hereby affirm that all information on my or my child(ren)'s Eastridge Church registration is truthful and accurate. I also hold free from any and all liability Eastridge Church, the Eastridge Staff, its respective employees, volunteers, and members, and do hereby for myself, my heirs, executors, and administrators, waive and release and forever discharge any and all rights and claims for damages which I may have hereafter accrued to be arising out of or in connection with my and/or my child(ren)'s participation in any of the activities of the Eastridge Church event.

I DO hereby consent and authorize Eastridge Church and Eastridge staff to take any and all action, including use of medical services and hospital facilities as they deem appropriate in the event that me and/or my child(ren) should become ill or otherwise injured under the care of the Eastridge program. I agree to hold harmless Eastridge Church and Eastridge Staff from all claims of injury, which may be sustained by me or the above child(ren) while participating in any Eastridge Church sponsored activity, whether caused by the negligence of the Eastridge Staff or otherwise. If medical attention is required, I give my consent for such medical care.
